What is Fibromyalgia?

Fibromyalgia is a condition, which causes muscular pain. This is accompanied by the softening of tissues around joins, causing a lot of pain and discomfort near the bone joints. Fatigue is another common symptom associated with this condition. Stiffness and sleep disorders are also common symptoms of this disease. The ?tender areas? of the body, such as the hip joints, the elbows, the knees and the area around the neck are the most affected regions.

This is a bizarre condition. A doctor makes a diagnosis when a person has had pain in at least 11 of the 18 characteristic tender points for three months or longer.

Symptoms of Fibromyalgia

1. Many people confronted with the disorder experience an intensification of fibromyalgia symptoms over time. In most patients, the fibromyalgia symptoms also tend to spread throughout the entire body.

2. Pain is the most prominent symptom of Fibromyalgia. It’s widespread—you can feel it from the head down to the knees—and pain becomes more severe when pressure is applied. In some patients, the painful body part becomes stiff and difficult to move.

3. Difficulty sleeping and experiencing extreme fatigue are also signals. These experiences are some of the common symptoms, and they can cause an extreme disruption in your life.

4. Yet another of the “common” symptoms many people with the disorder suffer from and experience are intense levels of depression and anxiety.

5. Other common symptoms of Fibromyalgia are Irritable Bowel Syndrome, which includes diarrhea and constipation; migraine; facial pain; painful menstrual periods; tingling sensations; chest pain; increase in sensitivity and dizziness.

Causes of Fibromyalgia

1. What causes fibromyalgia is still a mystery. Researchers believe it has something to do with the way the brain and central nervous system (CNS) interprets pain. Elevated levels of certain neurochemicals in the CNS have been noted in people with fibromyalgia.

2. Viral infections have also been blamed for the development of this disease in individuals. A number of scientist claim that the disease may also be caused due to food allergy from food products such as fermented foods, eggplant, dairy products, wheat and potatoes.

3. Sleep disorders are also suspected to be causes of fibromyalgia. Statistics indicate that more than 95 percent of people with fibromyalgia suffer from sleep apnea or insomnia. Until recently, scientists believed that sleep disorders were the consequence of fibromyalgia but it now turns out to be the other way around.

4. Injury can also lead to the development of fibromyalgia in some people in particular injury to the upper spine or injury that affects your central nervous system.

5. Another cause of fibromyalgia appears to be poor muscle oxygenation. Medical scientists claim that inappropriate oxygenation of the muscles is a plausible explanation for symptoms such as muscular weakness and stiffness, similar to the manifestations of rheumatoid arthritis. Although medical scientists haven’t yet unveiled the factors that prevent the oxygenation of the muscles, future research might provide medicine with an answer to the puzzle.

Fibromyalgia is often referred to as the “Invisible” illness or “Imaginary” disease. It was not until 1992 that it became recognized as a true medical problem by the World Health Organization. There are a few reasons for this. Some say it’s because it is often very hard to diagnose since Fibromyalgia’s symptoms often mimic those of other diseases and illnesses. Others say that it took so long for the medical community to recognize the syndrome because over 80% of the people affected have been women. For decades doctors have been telling patients with Fibromyalgia that they are just hypochondriacs.

What is Fibromyalgia?
Fibromyalgia syndrome is a common and chronic disorder characterized by widespread muscle pain, fatigue, and multiple tender points. The word fibromyalgia comes from the Latin term for fibrous tissue (fibro) and the Greek ones for muscle (myo) and pain (algia). Tender points are specific places on the body – on the neck, shoulders, back, hips, and upper and lower extremities – where people with fibromyalgia feel pain in response to slight pressure. Although fibromyalgia is often considered an arthritis-related condition, it is not truly a form of arthritis (a disease of the joints) because it does not cause inflammation or damage to the joints, muscles, or other tissues. Like arthritis, however, fibromyalgia can cause significant pain and fatigue, and it can interfere with a person’s ability to carry on daily activities. Also like arthritis, fibromyalgia is considered a rheumatic condition. You may wonder what exactly rheumatic means. Even physicians do not always agree on whether a disease is considered rheumatic. If you look up the word in the dictionary, you’ll find it comes from the Greek word rheum, which means flux – not an explanation that gives you a better understanding. In medicine, however, the term rheumatic means a medical condition that impairs the joints and/or soft tissues and causes chronic pain.

In addition to pain and fatigue, people who have fibromyalgia may experience:
• sleep disturbances
• morning stiffness
• headaches
• irritable bowel syndrome
• painful menstrual periods
• numbness or tingling of the extremities
• restless legs syndrome
• temperature sensitivity
• cognitive and memory problems (sometimes referred to as “fibro fog”)
• a variety of other symptoms

Fibromyalgia is a syndrome rather than a disease. Unlike a disease, which is a medical condition with a specific cause or causes and recognizable signs and symptoms, a syndrome is a collection of signs, symptoms, and medical problems that tend to occur together but are not related to a specific, identifiable cause.

How Is Fibromyalgia Diagnosed?
Research shows that people with fibromyalgia typically see many doctors before receiving the diagnosis. One reason for this may be that pain and fatigue, the main symptoms of fibromyalgia, overlap with many other conditions. Therefore, doctors often have to rule out other potential causes of these symptoms before making a diagnosis of fibromyalgia.

Another reason is that there are currently no diagnostic laboratory tests for fibromyalgia; standard laboratory tests fail to reveal a physiologic reason for pain. Because there is no generally accepted, objective test for fibromyalgia, some doctors unfortunately may conclude a patient’s pain is not real, or they may tell the patient there is little they can do. A doctor familiar with fibromyalgia can make a diagnosis based on two criteria established by the American College of Rheumatology (ACR). The criteria include

A history of widespread pain lasting more than 3 months The presence of tender points

Pain is considered to be widespread when it affects all four quadrants of the body; that is, you must have pain in both your right and left sides as well as above and below the waist to be diagnosed with fibromyalgia. ACR also has designated 18 sites on the body as possible tender points.

For a fibromyalgia diagnosis, a person must have 11 or more tender points. One of these predesignated sites is considered a true tender point only if the person feels pain upon the application of 4 kilograms of pressure to the site. People who have fibromyalgia certainly may feel pain at other sites, too, but those 18 standard possible sites on the body are the criteria used for classification.

The first step in getting suspected fibromyalgia diagnosed is to find a physician, nurse practitioner or other health care provider who is knowledgeable and compassionate. Maybe it should be the other way around: compassionate and knowledgeable.

The only sure way to find the right health care provider is by trial and error. You can narrow the field by talking to other people, especially people with fibromyalgia or who suffer some kind of chronic pain, and asking for recommendations. Rheumatologists are specialists in treating fibromyalgia, but many times an internist or family doctor is just as knowledgeable and helpful as a specialist.

Make an appointment with your chosen health care provider and tell him or her your story. Describe your pain and other symptoms and tell her how long you have had them. Answer their questions, and get a sense for how you are being treated. If you feel disrespected, or that you are not being taken seriously, make an appointment with a different health care provider. Keep going until you find a professional that you can relate to. Diagnosing and treating fibromyalgia is a partnership between you and your healthcare provider, and you need to be able to work together on this.

You can expect that a health care provider who is knowledgeable about fibromyalgia will take a health history, perform a physical examination, do some blood work and perform a point tenderness test. That’s the minimum.

The blood work will usually be to make sure there aren’t other problems, like rheumatoid arthritis, causing your symptoms. The real “test” for fibromyalgia is trigger point tenderness. The doctor will press on 18 specific areas of your body to test for tenderness. If you have achiness all over and are unusually tender over 11 or more of these trigger points, they will usually conclude that you have fibromyalgia.

Even this is a bit tenuous, though, because, as you know, the pain of fibromyalgia comes and goes. There are many other trigger points over your body that may be tender. The ones that are used for diagnosis were chosen by researchers as the ones most commonly affected.

Until recently, there have been no diagnostic tests for fibromyalgia, other than trigger point tenderness. A few tests are beginning to be used now, though. Your doctor may or may not order these tests; they are not widely used yet and their usefulness hasn’t been proven. The tests are:

* Cytokine levels, which are often abnormal in people with fibromyalgia.
* APA assay (antipolymer antibody). APA levels are thought to be elevated in people with fibromyalgia.
* Sleep study. This may be done both because most FMS patients have sleep abnormalities, so an abnormal sleep test makes the diagnosis clearer, and to identify and treat specific sleep problems.
* MRI or PET scan. People with fibromyalgia have abnormalities in both MRI and PET scan.
* EEG. Some researchers have discovered EEG abnormalities in people with fibromyalgia.

As more research is done, better, more definitive tests for fibromyalgia may become available. That would be a huge benefit for both patients and health care providers, because diagnosing fibromyalgia would be easier and more certain.
